• Commercial insurance with minimum $750,000 liability coverage

Driver Documentation

• Valid driver’s license (Class C or higher)

• Active work permit (if applicable)

• Social Security Number or ITIN

• Clean driving record (no DUI in the past 3 years)

Required Documents

• Certificate of Insurance (COI) listing Ironhawk Freight Solutions LLC as additional insured

• Vehicle registration

• Photo of the vehicle

• Signed agreement with our company

• Voided check or bank details for direct deposit

• Completed W-9 form
